Employee Handbook Acknowledgment
Company: Sunrise Pharmacy Group Handbook: Employee Handbook, version 4 (October 2026) Employee: Selorm Agbeko, Pharmacy Assistant, Spintex Road branch
Acknowledgment
I confirm that:
- I have received a copy of the Employee Handbook, version 4 (October 2026), either on paper or electronically;
- I have had the opportunity to read it and to ask questions about anything I did not understand;
- I understand that the policies in the handbook apply to me, including the policies on attendance and lateness, handling of prescriptions and patient data, cash handling, and the code of conduct;
- I understand that the handbook describes policies and procedures and is not a contract of employment, and that the company may update it, with notice of significant changes; and
- I know that questions about the handbook can be raised with HR at hr@sunrise.example or extension 204.
I understand that breaking the company's policies may lead to disciplinary action, as described in the handbook. Where the handbook and my employment contract differ, my contract applies, and I will ask HR whenever a policy is unclear to me.

About the Employee handbook acknowledgment form
A handbook only protects a business if you can show each employee received it. When a dispute turns on a policy, such as lateness, cash handling or how patient data is treated, the first question is usually whether the employee knew the rule. This form answers it.
The employee confirms which version of the handbook they received, that they had time to read it and ask questions, that the key policies named on the form apply to them, and that the handbook is not a contract and may be updated. Naming the key policies matters: it draws attention to the ones you most need understood.
Send the form with the handbook to every new hire, and again to everyone when you publish a new version. Salsox records when each person opened and signed it.

Questions
Is an employee handbook a contract?
Usually not, and the form says so: the handbook sets policies that can change, while the employment contract holds the binding terms. Mixing the two can make every policy update a contract change.
Do employees need to sign again when the handbook changes?
It is good practice for significant changes. Send the acknowledgment with the new version so you have a record for that version.
What if an employee refuses to sign?
Note that the handbook was given and the date. Policies generally still apply once communicated, but follow local employment law and keep the record.
