About the Child travel consent form template

Children travelling across borders without both parents, or with an adult who is not their parent, are sometimes asked for proof that the absent parent agrees. It is a safeguard against parental abduction and trafficking, and border officers and airlines can delay or refuse travel without it. A clear consent letter avoids that.

This form identifies the child, the parent giving consent, and the adult travelling with them, with passport details for both, the destinations, dates and flights, and where the child will stay. It gives a contact number for the parent that officials can call, and authorises the accompanying adult to consent to emergency medical treatment, with any allergies or conditions noted.

Requirements differ by country. Some ask for the consent to be notarised or to use a specific official form, and some require consent from every parent with parental responsibility. Check the rules of the countries the child is leaving and entering. A signed electronic copy is useful to carry, but bring a printed copy too.

Questions

Is a child travel consent letter legally required?

It depends on the countries involved. Some require one, others recommend it. Even where it is not required, it can prevent delays and questions at the border.

Does it need to be notarised?

Some countries and airlines ask for notarised consent. Check before travel; if it does, have the parent sign in front of a notary as well.

Who should sign if parents are separated?

Every parent with parental responsibility who is not travelling should consent. Add a second parent as a signer if needed.
