1. What this covers
Photographs, video and audio recordings of the participant taken during a photo and video shoot of a spa treatment for the spa's 2027 campaign, on 20 November 2026 (the "Material").
2. Permission
The participant gives Eden Wellness Spa permission to use, edit and publish the Material in the spa's website, social media accounts, printed brochures and in-store screens, for three years from the date of the shoot. The Material will not be used in a way that is misleading or that the participant would reasonably find offensive, and will not be sold to third parties.
3. Compensation
In return the participant receives a complimentary full-day spa package. No other payment is due for the use described above.
4. Withdrawing consent
The participant may withdraw consent for future use at any time by writing to marketing@edenwellness.example. The organisation will stop new uses and remove the Material from its website and social media within 30 days. Printed materials already distributed cannot be recalled.
5. Personal information
The organisation will keep the participant's details only to manage this consent, and will not share them except as needed for the uses above.

About the Photo and video release form template
Using a recognisable person's photo or video in marketing without their permission can breach privacy and data protection laws, and in some countries their image rights. A release form is the simple fix: written, specific consent signed before the content is used.
This template is specific where it counts. It names the shoot, lists where the images may appear, sets a time limit instead of "forever", and states any compensation. It also explains how to withdraw consent for future use, which data protection laws in many countries require, while making clear that printed materials already distributed cannot be recalled.
