About the Parental consent form template

Every trip outside school needs a parent's informed consent, and the form that records it should give parents what they need to decide: where, when, how students travel, who supervises, what it costs. This template puts those details first.

It then collects what staff need on the day: medical conditions and medication, allergies and dietary needs, an emergency contact, and consent for emergency medical treatment if a parent cannot be reached. Photo consent is a separate yes or no, because many parents are happy for their child to go on the trip but not to appear on the school's social media.

Sent through Salsox, parents sign on their phones in minutes, the school sees who has not signed yet, and the completed forms travel with the teacher in charge.

Questions

What should a parental consent form include?

The activity, date, transport, supervision and cost; the child's medical information and allergies; an emergency contact; consent for emergency treatment; and a separate choice about photos.

Can parents sign consent forms electronically?

Yes. Electronic signatures are widely accepted for school consent forms, and they make it easy to see who has not yet replied.

Why ask for photo consent separately?

Consent to a trip and consent to publishing a child's photo are different decisions. Keeping them separate respects parents' choices and data protection rules.
